Signing up for family support with Help Me Grow is easy! Three easy steps and you're on your way.....

Are you receiving Help Me Grow services? If so, skip to step 2. To see if you are eligible for Help Me Grow services, please call (216) 736-4300.

Next, request family support by filling in a Family Support Options Form. You can get this form by downloading it here, or by requesting it from your Service Coordinator. After you fill it in, the form can be mailed in, faxed, or given to your service coordinator. To get a Family Support Options form mailed to you, please call (216) 391-6096.

Now it's time to choose your Family Support! Family support is completely confidential and free of charge. Your options range from parent-to-parent support (Family Support Specialists), information about workshops, Web sites, playgroups, premature baby development information, as well as newsletters like FIN Facts. Choose as many of the supports your family needs. And if your needs change, you can add or drop services at any time. It's up to you.
